New Patient Forms

Welcome to PermaDent, a dental implant center in Torrance. We’re happy to have you in our dental family. These forms will help us give you the best possible care, and filling them out before your appointment will help us serve you and all our patients in a timely fashion. Don’t forget to contact your general dentist if you want them to send us supporting documents. If we don’t have the necessary imaging, we will either have to take new images in our office or delay your appointment.
